SWIM INTAKE AND THERAPY FORM NEW CLIENT

INDEMNITY, RELEASE and WAIVER

I confirm that I am the owner or person responsible for the dog(s) brought into South Paw Massage & Wellness/VBK9CC. I give consent for my dog to partake in services provided by South Paw Massage & Wellness/VBK9CC. In compliance with Florida State Law, I understand that South Paw Massage & Wellness/VBK9CC is not a licensed veterinarian, does not diagnose or cure specific ailments, or prescribe medications. I also understand that the services provided are not a replacement for proper veterinary care. Any injuries or diseases must be medically diagnosed and treated by the dog’s veterinarian. I am responsible for making South Paw Massage & Wellness/VBK9CC aware of any illnesses or injury’s that my dog has/had, or anything else that should be made aware.
I understand services provided may consist of activities such as swimming, stretching, muscle therapy, exercise and touch. Each session is dependent on things such as the condition and age of the dog, the goals of the owner, the nature of the dog’s injury (if any) and how the dog emotionally reacts to the environment.
South Paw Massage & Wellness/VBK9CC is not responsible for any injuries or illnesses incurred by myself or my dog(s). I agree to indemnify and hold harmless South Paw Massage & Wellness/VBK9CC from any such claims resulting from the use of the pool and deck.
I also understand that South Paw Massage & Wellness/VBK9CC is not responsible for any damages caused by my dog(s) to persons or to any property. I agree to indemnify and hold harmless South Paw Massage & Wellness/VBK9CC and its owners and employees from any such claims.
I have read, understand and adhere to the Disclaimer, Rules and Policies and the content form.
I agree that payment is due in full at the time of the appointment

There will be a “poop fee” of $80 if your dog happens to poop in the pool. The reason for this is because the pool will need to shocked, cleaned & disinfected. During this time, all appointments for the next 24-48 hours will have to be rescheduled.

****CANCELLATION POLICY: PLEASE CALL TO CANCEL ATLEAST 3 HOURS BEFORE YOUR APPOINTMENT TO AVOID BEING BILLED ½ THE FEE. NO CALL/NO SHOWS WILL BE BILLED FOR HALF OF THE SESSION COST.
